One group per client
Every host, certificate and domain watched for a client lives in its own group, with its own thresholds and channels.

Invite the client as a viewer scoped to their group
A scoped viewer sees only the groups they are invited into, so a client can watch their own status without seeing any other client.

Turn on the monthly report
Owners and admins get an emailed uptime report on the 1st for each group, with an optional SLA target.

Put the Beacon on the client's server
A plain script the client's own scheduler runs; it feeds every heartbeat, inside ping, status report and certificate report on that host from one pasted config.

Can a client see only their own group?

Yes. A viewer can be scoped to exactly the groups you invite them into, so a client sees their own group and nothing else.

Do we need to install something on a client's server for their heartbeats?

No. The Beacon is a plain script the client's own scheduler runs; it calls hostnag's ping, status report and certificate report URLs from one pasted config.
